Your customer reference number is of 10 digits that appears above your name on the address label. Type in the first 10 digits in the customer reference number field on the request form.
For example from the label below you will type 918104041 in the customer reference number field on the request form.



Incase you don't have this label handy, we will find it for you. Now go back to the request form by clicking "Back" button fill in as many details as possible and submit the form.
